Electric Gate Fatality
A Victorian firm Gate Automation Systems has been fined $350,000, after a technician took away critical parts of an electric gate and only left verbal safety instructions.
The technician removed the faulty motor from the 1.1T electric sliding gate for repair, leaving the gate in a state where it could run beyond the support posts and fall off the rails.
Two days later a worker leaving the depot was crushed and killed under the gate as he manually pulled the gate closed.
